PARTICIPATION AGREEMENT
I authorize Craig Duswalt International, Inc. to charge my credit card(s) as indicated in this enrollment form for a one-year membership in the exclusive RockStar Xcelerator MasterMind Program.
I understand and commit to working with total integrity, honestly and confidentiality with other members of this RockStar Group of like-minded top achievers.
I understand, and agree that, by completing and signing this enrollment form, I will have my credit card charged (or check cashed) the entrance fee rate described on the enrollment form for my inclusion in this Private and Exclusive Program.
I understand and agree that by completing and signing this enrollment form that I am enrolled in the RockStar Xcelerator MasterMind Program, and I am committing to a full twelve (12) month period in the program from the date entered by me on this enrollment form.
I further understand and agree that, if for any reason, I choose to remove or cancel myself out of the program prior to the end of the end of the twelve (12) month program dates, I am obligated to pay or continue paying any outstanding balance(s) for the entire twelve (12) month period from the date I entered on this enrollment form. Further, Craig Duswalt International, Inc. has sole discretion to terminate this agreement and remove any participant from continuing in the program at any time without a refund if the participant is disrupting the program, or becomes difficult to work with, or ceases to follow the RockStar Xcelerator MasterMind Program guidelines. If you are asked to leave, there will be no refund to the participant, and monthly payments will still be due until the outstanding balance is paid in full.
You may only cancel this transaction at any time prior to midnight of the THIRD (3rd) BUSINESS DAY after the date of this transaction. To cancel this transaction, submit a written Cancelation Notice by mailing a letter to the address below.
